This case series presents initial outcomes from the application of the ZAP-X stereotactic radiosurgery (SRS) system for treating spinal tumors. The study involved three patients diagnosed with spinal tumors, including schwannoma, meningioma, and spinal metastasis, who received treatment between February and June 2024. The ZAP-X system is characterized by its self-shielded, non-isocentric design, which aims to enhance treatment accessibility and efficiency. All patients underwent single-session treatments without acute complications or radiation-related side effects. Notably, one patient with spinal metastasis experienced significant pain relief, while another showed complete resolution of neurological symptoms with no tumor progression at follow-ups. The findings suggest that ZAP-X SRS is a safe and effective option for spinal tumor management, with potential advantages in both definitive and palliative care settings. Further studies with larger cohorts and extended follow-up are recommended to validate these initial results and assess the long-term efficacy of the ZAP-X system.
