This case report presents the initial experience of three patients treated with the ZAP-X stereotactic radiosurgery (SRS) system for arteriovenous malformations (AVMs) at Jersey Shore University Medical Center. The report outlines the characteristics of the patients, including their medical histories and the specifics of their AVMs, such as size and location. Treatment was conducted between March 2024 and September 2024, with dosimetric parameters and treatment quality metrics assessed. The patients received varying doses of radiation, and early follow-up imaging indicated either a reduction or stability in AVM volume, with symptom resolution observed in two patients. The report details the technical advancements of the ZAP-X system, including its gyroscopic design and real-time imaging capabilities, which enhance treatment precision. The findings suggest that ZAP-X SRS is a feasible and safe option for AVM treatment, although further prospective studies are necessary to evaluate long-term outcomes and obliteration rates.
