This case report details the application of ZAP-X stereotactic radiosurgery (SRS) for treating a primary optic nerve sheath meningioma (ONSM). The patient, a 68-year-old woman, presented with a 0.27 cm3 enhancing lesion along the left optic nerve, leading to proptosis and prior vision loss. The treatment involved a carefully planned SRS procedure using the ZAP-X platform, which is noted for its innovative design and efficiency in delivering targeted radiation. The treatment plan included six isocenters and a prescription dose of 2500 cGy administered in five fractions. Post-treatment evaluations showed significant improvements, including a 44% reduction in tumor volume and resolution of ocular symptoms. The report emphasizes the need for further studies to assess the long-term efficacy of the ZAP-X platform in managing ONSM, highlighting its potential as a preferred treatment modality over traditional surgical approaches due to lower risks of complications.
